Contract Duration مدة العقد

Case Worker will be responsible for the identification, assessment of protection issues, development and implementation of case plans including full documentation of cases, individual psychosocial care, referral (to relevant services) and follow-up of vulnerable and at-risk children and families.

The Case Worker will support children and their families through the provision of psychosocial support and provide direct support to children and their families from identification to case closure, based on principles of the best interest of the child. The Case Worker will refer cases to other available services, based on the different cases.

Functions

1. Provision of Intervention and Psychosocial support

• Identify individual cases, assess protection risks, and formulate case plans.
• Document all cases using agreed-upon forms and databases, ensuring confidentiality of the data collected.
• Ensure that all services and action points listed in the case plan are carried out within agreed time frames. Ensure that progress is regularly reviewed.
• Ensure regular follow up and support to children and women through home visits, provision of psychosocial support, referrals, and access to services.
• Adhere to standard documentation processes and follow best practice guidelines.
• Establish trusting relationships with children and women.
• Participating in training, team meetings and case conferences as requested by project manager.
  2. Referral with service providers, external partners, and child protection community groups
• Coordinate with other organizations/agencies and make referrals to specialized services.
• Establish continuing rapport/trust-building and interaction with the community and the social environment of vulnerable children and women for their protection, care, and development
• Conducting regular visits to children schools to work with teachers and facilitators on the identification and referral of at risk-children
  3. Reporting
• Provide weekly reports to the Project Manager.
• Keep-case files, records, and documentation up to date, secure and confidential.

4. Compliance

• Ensure compliance with policies and protocols of confidentiality, non-discrimination, consent, respect, and safety.
• Ensure that beneficiaries are duly informed on the Complaint Mechanism and can give their feedback on case management activities

الوصف الوظيفي لأخصائي الحالات

سيكون العامل مسؤولاً عن تحديد و تقييم القضايا المتعلقة بالحماية  و توسيع و تنفيذ خطط الحالة بما في ذلك التوثيق الكامل للحالات  و الرعاية النفسية الاجتماعية لبعض الحالات الفردية فضلاً عن نظام الاحالة( للخدمات ذات الصلة)  بالاضافة الى متابعة الاطفال و العوائل الضعيفة و المعرضة للخطر.

سيقوم العامل الاجتماعي بدعم الاطفال و اسرهم من خلال توفير الدعم النفسي و الاجتماعي  و تقديم الدعم المباشر للاطفال و اسرهم ابتداء  من تحديد الهوية  الى ان تنتهي الحالة  بناءً على المبادئ التي تكون في مصلحة الطفل و تثير اهتمامه.

علاوة على ذلك, سيحيل العامل الاجتماعي بعض الحالات الى  بعض الجهات المتاحة الاخرى بناء على نوعية الحالة.

الوظائف

1. توفير الدعم النفسي و الاجتماعي

• تحديد الحالات الفردية و تقييم مخاطر الحماية فضلاً عن صياغة خطط الحالات.
• توثيق جميع الحالات باستخدام النماذج و قواعد البيانات المتفقة عليها و كذلك ضمان سرية البيانات التي تم جمعها.
• التاكد من تنفيذ جميع الخدمات ونقاط العمل الموجودة في خطة الحالة ضمن الاطار الزمني المتفق عليه و كذلك يجب التأكد من مراجعة الخطة بانتظام.
• ضمان متابعة و دعم الاطفال و النساء بشكل مستمر من خلال الزيارات المنزلية و توفير الدعم النفسي و الاجتماعي بالاضافة الى الاحالات و امكانية الحصول على الخدمات.
• الالتزام بعمليات التوثيق حسب ما هو متفق عليه, و اتباع افضل الارشادات.
• اقامة علاقات مبنية على الثقة مع الاطفال و النساء.
• المشاركة في التدريب و اجتماعات الفريق و المؤتمرات المرتبطة بالحالة بناءٍ على طلب مدير المشرو.

2. الإحالة مع مقدمي الخدمة والشركاء الخارجينومجموعات المجتمع لحماية الطفل

• التنسيق مع المنظمات / الوكالات الأخرى وإحالة الخدمات المتخصصة
• إقامة علاقة مستمرة / بناء الثقة والتفاعل مع المجتمع والبيئة الاجتماعية للأطفال والنساء الضعفاء من أجل حمايتهم ورعايتهم و تطويرهم ايضآ.
• القيام بزيارات لمدارس الأطفال بشكل منتظم للعمل مع المعلمين والميسرين لتحديد وإحالة الأطفال المعرضين للخطر.

3. إعداد التقارير

• تقديم تقارير أسبوعية لمدير المشروع.
• حفظ الملفات  والسجلات  والوثائق محدثة وسرية و امنة. 

4. الشكاوي

• تاكيد الامتثال لسياسات وبروتوكولات السرية وعدم التمييز, الموافقة, الاحترام والسلامة.
• التأكد من إبلاغ المستفيدين عن  آلية  نقديم الشكاوى ويمكنهم تقديم ملاحظاتهم حول أنشطة إدارة الحالات

Education degree in relevant field

شهادة التعليم في احدى المجالات ذات الصلة
